The 5th-Grade Committee is fundraising for the amazing 5th graders to help make their last year a memorable one. We are going to be selling after-school treats on select Fridays 3:30pm-4:00pm. 

We'd love your input to see what your child prefers for a treat so we can have enough on hand but not over-purchase.  

This form will help us if we can get an idea of the interest for each item.
