TB Screening for Volunteers
There will be an MD parent available to sign/certify TB Assessment forms, if needed, this evening at the GO Green Costume Swap at Edison from 5-6 p.m. (THANK YOU!) The required form is attached. Please see below if you will need this form as an Edison PTA Volunteer. Also, stay tuned for an opportunity to get LiveScan fingerprinting done on campus next week!
If you plan to volunteer on a regular basis for the entire school year or have frequent and prolonged contact with students, you must provide the school site with a doctor’s certification that he/she was examined within the past 60 days and that he/she has tested negative or have been screened for risk of tuberculosis. Roles at Edison that require the TB Screening/Test are: garden, art docent, in-classroom volunteering, field trip chaperones, motor fitness.
