Dear Parents,
The end of the school year is fast approaching and we would like to thank our teachers and staff for all their wonderful work with our children through a centralized gift collection. Participation to the centralized gift collection is completely voluntary.
Teachers and staff will be able to choose their favorite gift cards from a large selection of SCRIP retailers. You can make a contribution in any amount to any teacher or staff you wish to recognize using the attached form.
Deadline is Wednesday, May 31st by 5pm. Please read carefully the instructions below to ensure your contribution is received on time and included in the centralized gift collection.

Submit your contribution using 2023 EOY Teacher-Staff
You should receive an e-mail to confirm that your contribution has been submitted.
If you do not receive the confirmation e-mail, please reach out to the PTA Team pta@fassv.org

Please make check out to FASSV-PTA and be sure to place it in an envelope labeled "FASSV-PTA/Teacher & Staff Gift Contribution" and drop off at the front desk or in outside mailbox. Thank you! - PayPal
Due to a change in PayPal fee structures that occurred over the summer, the PTA can no longer accept payments via PayPal without incurring a fee.and check the box to cover the fee.
However, if for convenience, you would still prefer to pay by PayPal, you may do so provided you are willing to pay the (nominal) associated fee. Please use the following link:
PLEASE DO NOT SEND PAYMENT DIRECTLY VIA THE PAYPAL APP. If you prefer not to pay the additional fee, please submit payment via check. We very much appreciate your cooperation in this matter!

Plan ahead; send your form and payment as early as possible to ensure it will be included. Receiving a bunch of forms last minute puts undue burden on our parent volunteers as we need to allow enough time for shipping and assembly of the cards before the winter break.
