April 12: First Day of Phase 2 On-site Learning

Here is a link to the Welcome Back Parent Information we reviewed and discussed. We have added a lot of new information so please review this document carefully.
Must Do Checklist Before The First Day of School:
Please complete the Onsite Instruction Opt-In Form. This needs to be done for each child attending onsite instruction at Holmes.
- Each day your child is on site they will need to hand the Daily Symptom Checklist form to their teacher. Best practice is to have your child enter campus holding the completed checklist in hand. This checklist is intended to be conducted at home by families prior to students attending school. Forms were provided in the distribution bags, or you can download and print a copy here: Daily Symptom Checklist. Copies will also be available outside of the office.
Medications at School: If your child requires routine or emergency medications while at school, your child's physician will need to complete the appropriate form(s). You will also need to sign it before it is valid. Physician's Recommendation for Medication; Asthma Action Plan; Return the signed forms and medications to the Health Office when your student returns to campus on April 12th. Over the Counter medications will not be dispensed for the remainder of this school year. If you have any questions, please email school nurse, Charlene Pattillo email@example.com
We are pleased to announce ALL classrooms, TK - 5, will be offering Tutoring/Enrichment Monday - Thursday from 2:00pm - 2:55pm. Tutoring/Enrichment will begin on April 12th. Please complete the form ASAP, so we can plan accordingly. Please complete this form for EACH CHILD currently enrolled at Holmes. If you decide NOT to participate in tutoring dismissal will be at 2:00pm. Thank you!

Based on our Instructional Model Survey Results, 91% of our students wish to participate in the onsite/hybrid model. I am pleased to announce, we will be providing 4 days of on-site instruction for all our students who have opted to come on campus. Please note we cannot honor decisions to switch from online to on-site learning due to our space capacity.

I want to thank our teachers, office staff, and custodial staff, who have all worked tirelessly on room configurations to realize this opportunity for our students. I will continue to send communications as new information is available.
