Volunteer Paperwork update

Longfellow Families,

If you plan to volunteer in the classroom, volunteer on campus, teach Art Corps, or drive on a field trip this year, you must complete a volunteer application.  Applications must be submitted every year.  A copy of the application is attached and also available in the office.

 

This year you may either submit your application in the office on Wednesdays (now through December 5th), OR you may e-mail a copy of your completed application to Woodlyn Perez, Parent Volunteer Coordinator, at wperez@sandi.net.  Please be sure to include:

  • Completed School Volunteer application including Fingerprint form if driving on a field trip
  • Copy of your Driver’s License
  • Copy of TB Clearance
  • Copy of car insurance if you plan to drive on a field trip

The volunteer application includes the SDUSD School Volunteer Application, the Volunteer Code of Conduct and the SDUSD Request to Conduct Volunteer Screening (Fingerprint form).  All field trip drivers must complete the fingerprint process by School Police Services.  If you have already completed the fingerprint process you will not need to be fingerprinted again, but the Fingerprint form needs to be filled out again (please check “Returning Category D Volunteer”).

 

Volunteers must also provide a TB clearance card or have one on file in the school office (they are valid for 4 years). If you need a TB clearance card and your answers are all “No” to the questions on the attached Adult TB Risk Assessment Questionnaire form, the School Nurse is available to meet with parents on Tuesdays and Fridays to clear your TB compliance. Copies are also available in the office.
