I just wanted to touch base with you on the documents due within the next week or two. I have received checks so far from only 3 of our 13 PTA's. Please make sure you are working to get these to me. Please allow enough time for you to be able to gather two signatures on the live check prior to mailing. We (at Council) are planning on writing a check shortly after October 1st to Sixth District PTA, including all gathered member dues as well as insurance dues. Please get those to me ASAP, with a Unit Remittance Form attached. You can contact me for a mailing address.
All other docs may be sent via email, please make sure they are in PDF form (no excel or word docs please).
I have attached a list of the items needed. I have done my best to update the spreadsheet, please let me know if anything needs to be updated.
Member dues collected via the Totem system do not require any action from you, consider those dues paid. Any member dues collected manually will need to be submitted monthly via paper check. Please let me know if you have any questions : )
TAXES: Your 19-20 Treasurer should be working on these now. You will need to have your audit and financial report completed prior to completing the taxes, so please make sure these are completed first.
I have attached a copy of the whistleblower form. Please have each of your 4 main officers fill these out and return to me.
Also attached is the Workers Comp form. All our Milpitas PTA's have paid no one as an employee in the recent past. Unless you have hired a new employee, (presidents or treasurers) please complete with "no one paid" written across the middle.
Meetings Dates List doesn't need to be fancy, we just need a list of your meeting dates for the year in a word doc (saved as pdf).
