COVID Testing Consent

Good morning families:

 

The NYCDOE is encouraging families to submit consent for random COVID-19 testing which will resume the first week of school on a biweekly basis.

 

 

Families can re-submit a new COVID-19 testing consent form for the 2021–22 school year to participate in the testing program. If testing consent exists from the 2020–21 school year, that consent can still be used until it expires on September 30, 2021.

 

Families can consent in two ways: NYC Schools Account (NYCSA), or by submitting a signed paper consent form. If you need a paper copy, they will be available at the gate during drop off.
