Information about the DOE's in-school testing for the 2021-22 school year can be found here:
The short version of the testing plan is:
- Each school should be visited every two weeks to test up to 10% of unvaccinated students chosen from the group who has "consent for testing" forms on file
- PreK and K students are not included in this plan
- If you were in person last year, or for a summer program, you must submit a new COVID-19 testing consent form for the 2021-2022 school year to participate in the testing program. (For new students, the program is Opt IN, not Opt OUT.)
You can also print a consent form and return it to school. The English version is attached. It is also available in 9 other languages at the link above.
To consent online, visit the new NYC Schools Account (NYCSA) Portal (formally mystudent.nyc) at https://www.schoolsaccount.nyc/
Once you sign in, you should see your children's name(s) at the top. Click on the blue "My Student" box below that, select your child's name and you should see a green box at the top that takes you to the Covid19 consent form.
The direct link is https://mystudent.schoolsaccount.nyc/ConsentForms/CovidTestingConsent (but that only works once you are logged in.)
