Information about the DOE's in-school testing for the 2021-22 school year can be found here:


The short version of the testing plan is:

  • Each school should be visited every two weeks to test up to 10% of unvaccinated students chosen from the group who has "consent for testing" forms on file
  • PreK and K students are not included in this plan
  • If you were in person last year, or for a summer program, you must submit a new COVID-19 testing consent form for the 2021-2022 school year to participate in the testing program. (For new students, the program is Opt IN, not Opt OUT.)


You can also print a consent form and return it to school.  The English version is attached. It is also available in 9 other languages at the link above.

Once you sign in, you should see your children's name(s) at the top.  Click on the blue "My Student" box below that, select your child's name and you should see a green box at the top that takes you to the Covid19 consent form.
