As you know, each year the NYC DOE requires a new Emergency Contact Card for each child. It is critical we have current information for all students so we know whom to contact in an emergency.
Please fill out the attached form and email it to:
Make sure to include:
* Cell phone numbers and emails for all parents and guardians
* Phone numbers for three (3) additional adults who we could call if we can't reach you - friends, neighbors, caregivers, grandparents, aunts, uncles, cousins
* Any other information that would help us find you, and best help your child in an emergency
We are asking for these cards to be returned as soon as possible. Before the first day of school would be great and no later than Friday, September 14th - Please!
