This orientation is for families with students who will be coming to school inside the building on A or B days. You will learn about:

SCHOOL LUNCH FORM - CLICK HERE TO FILL OUT ONLINE

Filling out the school lunch form annually is as important as the CENSUS. Funding for PS 107 is directly tied to the responses received on the form and the participation by every family is crucial. It is especially important this year assuming that some families employment has been affected by Covid-19. There is an option on the form to opt out of disclosing your salary if you would prefer, but you still need to complete the form and enter information for all of your children in the NYC school system.  

 

MEDICAL FORMS 2020 - A note from Nurse Kara:

All NEW students to PS 107 must submit a completed medical exam form and provide proof of all required immunizations.  In response to COVID-19 pandemic, the Office of School Health (OSH) will accept forms based on physical exams performed with the previous 18 months. OSH will not accept a medical exam form that does not include the results of an in-person physical examination.  This will remain in effect until December 31, 2020.

 

For current PS107 students, a new medical exam form is only required if the initial examination was performed before the child was five years old.  All other students do NOT need to submit a new medical exam form each year.

 

Students who require medication at school (Asthma inhalers, Epi Pens, Benadryl, etc.) must have a medical provider complete the appropriate Medical Examination Form (MAF).  There are separate forms for 1) Asthma, 2) Allergies, 3) Seizures, 4) Diabetes and 5) all other medications.  MAF must be completed and signed by a medical provider.  In addition, a parent/guardian must sign the back of the MAF.
