Please make sure you have submitted the DOE Learning Preference Survey. We appreciate those who have recently emailed us their decision, but to make it official, we need you to submit this form.
For families who are have not yet notified us about plans to select all-remote but have made this decision as of Sept, 10th, here are the TWO steps we need you to take:
1. Fill out the DOE Learning Preference Survey
2. Also complete this PS 58 All-remote request as of Sept 10th (because the DOE data is sent to us every 3 days!)
