Final deadline extension to take the 2025 NYC School Survey is now Friday, May 2!  

Surveys can only be taken online at NYCSchoolSurvey.org from now through the extended deadline in any of the nine NYCPS-supported languages, using your survey access code. 

The family access code is lowercase “f” followed by your child’s nine-digit Student Identification (OSIS) Number. 

The deadline to return paper surveys was April 11 and are no longer being accepted.

NOTE:
Individual responses from families, students, and teachers are never shared with schools or NYCPS. Survey responses are confidential, so please provide your honest feedback!
