In order to volunteer in a classroom or chaperone a field trip, parents must be cleared  by the District. To be cleared, parents must be fingerprinted and a have a current tuberculosis (TB) clearance. At Jaguar Day, we will have a fingerprinting station to help parents complete one of the steps necessary to volunteer. 

The cost of  fingerprinting is $67 for parents who have resided in California for over a year and $84 for parents who have resided in California for less than a year. Credit card will be the only payment option accepted.

Attached is the Livescan form parents will need to complete and bring with them on Jag Day, along with the volunteer form to send into the District Office once all your documents are ready.

To ensure a smooth process, please bring completed Livescan form, credit card payment and moisturize your hands so fingerprints are readable.

You must submit the completed Livescan form, Volunteer Form and TB Clearance Form via email to personnelfrontdesk@mdusd.org. You will not be added to the master list of school volunteers until ALL required documents have been received at the district office, your fingerprints have been cleared by the Department of Justice, which can take 1-30 days.
