As some of our classrooms have their first field trips underway this month, we wanted to review district volunteer policies one more time as both field trip chaperones and drivers need to be cleared through the office.
Volunteer requirements for on-campus volunteering and field trip chaperoning:
πͺͺ Holmes Volunteer Application + copy of government issued ID
π TB Risk Assessment Questionnaire or copy of recent negative TB screening/test
Volunteer requirements for field trip drivers:
πͺͺ Holmes Volunteer Application + copy of driver's license
π TB Risk Assessment Questionnaire or copy of recent negative TB screening/test
π« Fingerprint clearance through LiveScan (completed off-site)
π» Completion of district-mandated online training course "Child Abuse: Mandatory Reporting" (~35 minutes to complete)
π Copy of up-to-date proof of car insurance
NOTE: A new Holmes Volunteer Application is required to be completed every year. TB Risk Assessment Questionnaire is required to be completed every 4 years. Fingerprint clearance is required only once and is good for all continuous volunteering years in the district. Online training module "Child Abuse: Mandatory Reporting" is required to be completed every year.
You can pick up and return all relevant forms, applications, and instructions at the front office. A school nurse is available on campus to complete TB Risk Assessment Questionnaires on Fridays.
Please contact Raquel Gonzalez rgonzalez4@sandi.net or stop by the office if you have questions.
